The California Consumer Privacy Act of 2018 (the “CCPA”) provides California residents certain rights regarding their Personal Information collected by Rehab Excellence Center.

For the purposes of the CCPA and this Point of Collection Notice (“Notice”), a Consumer is defined as an individual residing in the State of California, including visitors to rehabexcellencecenter.com, all subpages and communication capabilities accessible therein through messaging, chat, or similar functionality (collectively the “Site”) and those who are acting as a job applicant to, an employee of, owner of, director of, officer of, medical staff member of, or contractor of Rehab Excellence Center.  The CCPA defines Personal Information as information that identifies, describes, or could reasonably be linked with the Consumer (“Personal Information”).

This Point of Collection Notice (“Notice”) is intended to inform Consumers of the categories of Personal Information that we collect and the purposes for which those categories of Personal Information are used, as required by the CCPA.

The CCPA does not apply to the following categories of information:

  • Medical information governed by:
    • Confidentiality of Medical Information Act (CMIA)
    • Health Insurance Portability and Accountability Act (HIPAA)
    • Health Information Technology for Economic and Clinical Health Act (HITECH)
    • Genetic Information Nondiscrimination Act (GINA)
  • Privacy data covered by other regulations including:
    • US Gramm-Leach-Bliley Act
    • US Driver’s Privacy Protection Act
    • California Financial Information Privacy Act
  • Information collected by government entities

Purposes for Collecting Personal Information

Rehab Excellence Center collects your Personal Information for recruitment, employment, personnel related and other business purposes.

How Rehab Excellence Center Obtains Personal Information

Rehab Excellence Center may obtain Personal Information either directly from you (forms you complete through our website or through other online communications with Rehab Excellence Center), or indirectly from you (by tracking actions you take on our website). We may also obtain Personal Information from third-party vendors who perform business services on our behalf.

Sharing Personal Information

Rehab Excellence Center may disclose your Personal Information to third-parties, with your consent when required by federal and state law, including consumer reporting agencies for employment background checks, service providers with whom the Consumer directs us to share such Personal Information, and service providers that fulfill a specific business purpose (i.e. payroll companies, health/benefits/wellness providers, human resource consultants, staffing agencies, etc.).

In addition, we may use, sell, or disclose the Personal Information we collect for one or more of the following purposes:

  • To fulfill or meet the reason you provided the information. For example, if you share your name and contact information to subscribe to a newsletter, register for an event, or request information regarding other products or services then we will use the Personal Information to fulfill such request. If you share your name and contact information to request an appointment or ask a question about a Rehab Excellence Center service, we will use that personal information to respond to your inquiry. If you provide your personal information to complete job applications, seek employment opportunities, and to complete related information requests related to seeking such prospective job opportunities then we will use that information to facilitate completion of a potential employment opportunity.
  • To create, maintain, customize, and secure your account with us.
  • To personalize your Site experience and to deliver content and product and service offerings relevant to your interests, including targeted offers and ads through the Site, third-party sites, and via email or text message (with your consent, where required by law).
  • To respond to law enforcement requests and as required by applicable law, court order, or governmental regulations.
  • As described to you when collecting your personal information or as otherwise set forth in the CCPA.

Right to Amend Point of Collection Notice

This Notice supplements the information contained in our Privacy Policy. Rehab Excellence Center may update this Notice from time to time. When we do, we will post the Notice and revise the date at the bottom of this page. We encourage you to frequently check this page to stay informed about how we are helping to protect the Personal Information we collect. You acknowledge and agree that it is your responsibility to review this Point of Collection Notice periodically and become aware of any modifications.

A Message To Our Customers,

Rehab Excellence Center’s Commitment to Patients and Safety

The world has been facing an unprecedented challenge with the COVID-19 Pandemic. Like you, we have been deeply concerned and want to do what we can to help keep our customers, communities and families safe and healthy.

Rehab Excellence Center takes the responsibility to our customers, our teams and our patients very seriously and we are working to make sure our customers have access to our quality care in the safest manner possible. 

After listening to the appropriate sources for accurate and up-to-date information, we have been taking sensible steps to make sure our preparedness plan reflects those inputs.  We will continue to monitor the situation and make any changes to ensure a high level of safety for our patients. 

What We’re Doing:

  • Providing physical therapy services in a small and controlled visit setting (Single Therapist Office) and typically just 1-3 patients in the room during the patient visit. Following social distancing guidelines is a requisite.

  • Following CDC guidelines, we kindly require our patients to wear masks to protect the health and safety of our patients and staff during their physical therapy visits. 

  • Equipping each REC facility with a Non-Contact Infrared Forehead Thermometer, providing fast and reliable body temperature measurement (1 second testing).

  • We are encouraging all of our patients to thoroughly clean their hands with either our alcohol-based hand rub or wash them with soap and water (and for 20 seconds or longer each time). Washing your hands with soap and water or using an alcohol-based hand-rub kills viruses that may be carried on the hands.

  • Our employees are also working diligently to ensure everyone is following our existing protocols for hand-washing, disinfecting and sanitizing equipment, treatment tables, waiting areas, bathrooms, door knobs, and other general areas.

  • Per the World Health Organization (WHO), we suggest patients stay home if they feel unwell and we can re-schedule their appointment for a later date. If they have a fever, cough, or difficulty breathing, we ask they call REC in advance and seek medical attention. Calling in advance will allow their health care provider to quickly direct them to the right health facility and better protect them and help prevent the spread of viruses and other infections.
