Acceptance of Terms of Use

Rehab Excellence Center (“us”, “our”, or “we”) requires that all visitors (“you”) to the Rehab Excellence Center website (rehabexcellencecenter.com), including all subpages and communication capabilities accessible therein through messaging, chat, or similar functionality (collectively the “Site”), abide by the following Terms of Use (“TOU”).   Rehab Excellence Center operates clinics and otherwise provides services in various jurisdictions within the United States.  The Site is intended for use only by authorized users who are at least 18 years of age and who reside in the United States.

PLEASE NOTE: Your access to and use of the Site are subject to the following TOU, as well as all applicable laws and regulations. Please read these terms carefully. If you do not accept and agree to be bound by any of these terms or conditions you are not authorized to access or otherwise use this Site or any information, content, Services or Software contained on this Site. Your access to and use of this Site constitute your acceptance of and agreement to abide by each of these terms and conditions set forth below. These TOU may be changed, modified, supplemented or updated by Rehab Excellence Center from time to time without advance notice.

Reservation of Rights

Rehab Excellence Center owns the Site and Rehab Excellence Center, or its content providers, own and/or retain the rights to all content, Web pages, source code, materials, data, information, text, screens, services, design, layout, screen interfaces, “look and feel,” logo or graphic images appearing herein, and the operation of this Site (collectively, “Materials”), which are protected by various intellectual property laws, including, but not limited to, copyrights, patents, trade secrets, trademarks, and service marks.  Nothing in these TOU transfers any rights in the Materials to you or any third party and you acknowledge and agree that you do no not acquire any ownership rights by downloading or viewing any Materials.

Use of Site

As a condition of your use of the Site, you acknowledge and agree that you will not use the Site for any purpose that is unlawful or otherwise prohibited by these TOU.  You will not use the Site in any manner that could damage, disable, overburden, or impair the Site or any related service provider server, or the network(s) connected to the Site or any related service provider server, or interfere with any other party’s use and enjoyment of the Site.

A link to a third-party website (“Third Party Site”) on this Site does not mean that Rehab Excellence Center endorses or accepts any responsibility for the content, functioning or use of such Third Party Site, and you enter any such website at your own risk.  You agree that Rehab Excellence Center has no control over or liability for information on Third Party Sites.  You should be aware that Third Party Sites may contain rules and regulations, privacy provisions, confidentiality provisions, and other provisions that are different from the provisions provided on this Site.  Rehab Excellence Center is not responsible for such provisions, and expressly disclaims any and all liability related to such provisions.  Rehab Excellence Center prohibits unauthorized hypertext links to this Site, or the framing of this Site.

Privacy Statement

Our Privacy Policy and Point of Collection Notice describe Rehab Excellence Center’ collection, use and protection of personal information and will help you understand how we secure and treat personal information that we obtain. Your submission of any such information to Rehab Excellence Center in connection with the Site constitutes your consent to the uses and disclosures of data and information by Rehab Excellence Center or its affiliates that are consistent with the Privacy Policy and Point of Collection Notice.

Disclaimer of Warranties/Limitation of Liability

International Use

This Site is only directed to persons in the continental United States where Rehab Excellence Center provides services.  If you are located outside of the continental United States, you are directed not to use the Site.  Those who choose to access this Site from other locations do so on their own initiative and are responsible for compliance with local laws.

Miscellaneous

These TOU constitute the entire agreement between you and Rehab Excellence Center and supersede all prior or contemporaneous communications, promises and proposals, whether oral, written or electronic, between you and Rehab Excellence Center with respect to this Site.  If any part of these TOU is determined to be invalid or unenforceable pursuant to applicable law including, but not limited to, the warranty disclaimers and liability limitations set forth above, then the invalid or unenforceable provision will be deemed superseded by a valid enforceable provision that most closely matches the intent of the original provision, and the remainder of these TOU shall continue in effect.  A printed version of these TOU and of any notice given in electronic form shall be admissible in judicial or administrative proceedings based upon or relating to these TOU to the same extent and subject to the same conditions as other business documents and records originally generated and maintained in printed form.  All rights not expressly granted herein are reserved.

A Message To Our Customers,

Rehab Excellence Center’s Commitment to Patients and Safety

The world has been facing an unprecedented challenge with the COVID-19 Pandemic. Like you, we have been deeply concerned and want to do what we can to help keep our customers, communities and families safe and healthy.

Rehab Excellence Center takes the responsibility to our customers, our teams and our patients very seriously and we are working to make sure our customers have access to our quality care in the safest manner possible. 

After listening to the appropriate sources for accurate and up-to-date information, we have been taking sensible steps to make sure our preparedness plan reflects those inputs.  We will continue to monitor the situation and make any changes to ensure a high level of safety for our patients. 

What We’re Doing:

  • Providing physical therapy services in a small and controlled visit setting (Single Therapist Office) and typically just 1-3 patients in the room during the patient visit. Following social distancing guidelines is a requisite.

  • Following CDC guidelines, we kindly require our patients to wear masks to protect the health and safety of our patients and staff during their physical therapy visits. 

  • Equipping each REC facility with a Non-Contact Infrared Forehead Thermometer, providing fast and reliable body temperature measurement (1 second testing).

  • We are encouraging all of our patients to thoroughly clean their hands with either our alcohol-based hand rub or wash them with soap and water (and for 20 seconds or longer each time). Washing your hands with soap and water or using an alcohol-based hand-rub kills viruses that may be carried on the hands.

  • Our employees are also working diligently to ensure everyone is following our existing protocols for hand-washing, disinfecting and sanitizing equipment, treatment tables, waiting areas, bathrooms, door knobs, and other general areas.

  • Per the World Health Organization (WHO), we suggest patients stay home if they feel unwell and we can re-schedule their appointment for a later date. If they have a fever, cough, or difficulty breathing, we ask they call REC in advance and seek medical attention. Calling in advance will allow their health care provider to quickly direct them to the right health facility and better protect them and help prevent the spread of viruses and other infections.

We recognize this is a challenging time for all and we remain deeply committed to the safety of our customers, teams and communities.  Our patients and customers are the heart of our business and we thank you for your long-standing trust in Rehab Excellence Center, as we continue to navigate this uncertain period ahead.
